Licensing issues,laison with the referring clinicians and are known issues which are not impossible to address.
The other issues raked by sceptics especially in the NHS are patient confidentality when sending images over the net and delays in reports reaching hospitals.Isnt patient confidentality at risk when the radiologists pull images on their home computers?
Nobody talks about the quality of the reporting because at times its better!
Teleradiology will only grow as market forces equalize.
