Writing papers - KEM Hospital and a story of possibilities
Residents in India keep complaining about the lack of opportunity for writing articles. Excuses abound; no documentation, no experience or guidance, no access to previous articles, no access to "Instructions for Authors", no time, no secretarial help, etc...
Of these probably the only important issue may be that of guidance. If there are people who can guide in writing the article properly, I am sure the rest of the issues can get sorted out pretty easily.
KEM Hospital for example has always had a rich culture of writing. It is not that there are better cases seen at KEM than at the other institutes or that it has "better" residents. What it has is a culture of "writing" that all successive residents build upon.
To take just one example. One resident, Ashwin Asrani, has seven articles retrievable in a Pubmed search with his name. Two, are in the Journal of Postgraduate Medicine, which is a local KEM journal that is indexed. Just one article is a series report, the rest are case reports. Yet, he already has one article in Radiology and two in the Journal of Utrasound in Medicine.
There is no earth-shaking research that he has done...it is difficult to do good research in our departments. But with just case reports (which all institutes have by the dozen), perseverance and a good choice of journals, he has managed to get seven articles published and I am sure there are some more in the pipeline.
The reason for using Ashwin's example is to show that it is possible to get articles published internationally. And excuses to the contrary are just that....excuses...which as Indians, we are so great at making...whether it is journal writing or the Olympics. With Internet access, Pubmed is available everywhere. A large number of journals have free access to articles more than 1-2 years old. Many journals (for example the entire BMG group) have free full-text access from India. Computers have made the task of "writing" and "correcting" so much easier. Submission in most cases now is electronic, saving the Rs. 1000 odd that we had to spend on international courier charges. What more does one want? More excuses....?

I almost fell out of my chair when I read this. It is way too high praise from Dr Bhavin sir for which he rightly says " Not earth shaking ". Things will be more in perspective if you know that there have been several more residents before me (Hemant,Ranjeet, Arpit, Mohit to name a few) who have published so much more that we dubbed them as 'Printing Presses'.
There are a few facts I would like to lay forth as to why this is possible in KEMRAD. Most of them are already as Dr. Bhavin sir has said, but the second one I feel is very important as everyone knows it but few will come up openly about it. In order of importance they are
1)Presence of Illustrious and shining examples like Dr. RR sir who have published genuine stuff at a phenomenal rate in international journals before the era of PC's and the internet.
2)A democratic approach to publishing in the department by Dr. RR sir which allows one to publish right from the 1st post onwards while following the authorship criteria strictly. This is unlike other places where you do all the work and your HOD, AP. Lect etc names' appear in that order before yours or sometimes your name does not figure at all. Also RADRECOR (database) is a must for any series report.
3) A tradition of "Passing the torch" by seniors. I was initiated into "writing" by Govind Chavan and guided by several others. ( I am hopelessly addicted now)
4)Several other smaller factors figure in this too, like 24X7 free access to PC's and the internet, affinity for typing, knowledge of word and image processing software. These should not be problem in any urban or semi-urban area.
Another thing I would like to mention is that after writing a few case reports one should move on to writing series report and onto bigger things.
On the flipside, sometimes this "writing" thing does get out of hand at times and a common joke which does the rounds (authored by Shreyas) typifies this. This is how it goes
One mother to another " Aapka beta KEMRAD mein padhai (=studies)kar kar ke doctor ban raha hai ?" The latter replies " Nahin, nahin woh to KEMRAD mein likh likh kar (publishing) doctor ban raha hai.

One very senior head-neck imaging authority in the US fumed at the name of an ex-kemite now sitting pretty in the UK as a champion of plagiarised and fabricated data as a feature of all his publications. Well I'm sure the KEM guy has the last laugh as he is in a very plum career postion in the UK, but whether that makes him any more insightful and perspicacious in dealing with the patient's condition is a different matter altogether.Who cares, anyway??!!
But then to each his own.........,the only issue is whether this publication consciousness ought to be a desirable goal-above all else- for all residents, KEM or otherwise.It seems to me that the denouement of all medical prowess is in applying it for the patient's welfare.......which is merely a pipe-dream without learning the basics of that discipline.Only then can we truly edify and honour our calling-the subtle and sublime realm of diagnostic medicine, radiology.
